New Riders: Registration, Medical Form/Physician Release, Scholarship Application (if wanted), Scheduling, PT/OT Fomrs (if applicable), Liabilty, Photo Release, EMT, Payment Contract.
Continuous: Payment Contract, Schedule, and Scholarship (if wanted)
Non-continuous: Payment Contract, Schedule, Scholarship (if wanted), and Continuation
